What is a Superbill?

If you are a self-pay client that would like to be reimbursed for payments, you are encouraged to speak with your insurance company regarding their out-of-network coverage prior to beginning therapy. As an “out-of-network provider,” I can supply you with a superbill upon request. A superbill is a document you submit directly to your insurance company for potential reimbursement. Like claim forms, superbills include a list of services, procedure codes (i.e. CPT codes), payments/balance, and other pertinent information for your insurance company. Questions for Your Insurance Provider

To verify out-of-network coverage, call the number on the back of your health insurance card and ask for “Member Services.” The following questions may be helpful when discussing coverage with your insurance company.

• Do I have out-of-network coverage for speech/swallow/voice therapy services?

• Do I have out-of-network coverage for speech/swallow/voice therapy services provided through telehealth?

• What is my yearly deductible and has it been met? If not, how much more do I pay until my deductible is met?

• How many sessions per year does my plan cover?

• How much does my insurance plan reimburse for an out-of-network provider?

• Is approval from a physician or physician order required for speech/swallow/voice therapy?

• Is prior authorization required?

• Do I have a co-payment?

• What is the home and mailing address on file? (Ensure checks are issued to your correct address).

• How do I submit a superbill?

• What is the time limit to submit a superbill?
